Application forms are available under Resources and Links. There are SEVEN forms included in the application. These forms are interactive pdfs. You may type in the information before printing them or print the form to be completed by hand. The completed forms can be hand delivered to Cascia Hall or sent via mail, email, or FAX.
Form #1 - Student/Family Information
To be completed by parents and returned to the Office of Admissions.
Form #2 - Student Activities and Interests
To be completed by the student and returned to the Office of Admissions.
Form # 3 - Request for Records
To be completed and signed by parents and forwarded to the student’s current school. The school will send the records requested to the Office of Admissions. Cascia Hall requests one full year of grades (previous year) plus the report card from the most current grading period, recent standardized test results, and attendance records. Unofficial records are acceptable for application purposes.
Form #4 - Parent Waiver
Complete the “Recommendations - Parent Waiver” form and return it to Cascia Hall. An application will be incomplete without this signed waiver.
Forms #5 and #6 - Teacher Recommendations
Give these recommendation forms (#5 and #6) to two recent classroom teachers (at least one current). Ask them to return the completed forms directly to the school.
Form #7 - Pastor/Church Recommendation
Please give this form to your pastor, associate pastor, youth director, or Sunday school teacher. The recommender will submit the recommendation directly to the school. If for some reason a Pastor/Church Recommendation is unavailable, please write “not available” on the form and return it to the school. If we know the completed form is not coming, your application will be processed without it.
